Image -> Image Size...

Then set your width & height. By default it will keep the original aspect ratio (ratio between width & height), which will prevent any distortion of the image. Note that avatars do not need to be precisely 100x100, they just can't be more than 100 wide or 100 high.

If you want to force a non-square image to be square, uncheck "Constrain Proportions." If you want to cut a square image out of a non-square image, select the Marquee tool (it's the one on the far upper left of your left toolbar, looks like a dashed square), and hold down Shift while dragging across the image to make a perfectly square selection. Then select Image -> Crop. Finally, size the image to 100x100 with Image -> Image Size.

Use Save for Web instead of the regular save. If you're doing it as a GIF file, use "adaptive" color, and just reduce the number of colors until the size is small enough (size shows up near the bottom of that window), but keeping good image quality. Don't change the quality setting for GIFs.

For that photo, you may actually want to use JPG instead, though, around 60% quality.
